The $150 covers the health consultation, neurological assessment and the doctor’s report of findings. A $75 non-refundable deposit is due upon booking.

No Call, No Show Policy: If the appointment is rescheduled or cancelled within 24 hours of the appointment time, or if there is a no call/no show, the remaining $75 will be charged to the card. If another appointment is scheduled within 24 hours of the cancellation or no show, then the original deposit of $75 will be transferred to the new appointment.

You will receive a link to the new patient paperwork once your appointment has been confirmed. All paperwork must be completed at least 60 minutes PRIOR to scheduled appointment time. If paperwork is not completed 60 minutes prior to scheduled time, this appointment will be rescheduled and will be subject to our No Call, No Show policy.
